Based on document review and interview, the nursing staff failed to report a patient-staff physical altercation and complete an incident report per policy for 1 of 10 patient medical records reviewed. (P1)

Findings Include:

1. Facility policy titled "Incident Reports", PolicyStat ID: 13033981, last revised 01/2023, indicated any hospital staff member who witnesses, discovers or has direct involvement in and/or knowledge of an event must complete an incident report, or give a detailed report to the person completing the incident report.

2. Review of P1's medical record on 2/7/2024, daily nursing assessment indicated that P1 punched the staff nurse in the chest while the staff nurse attempted to draw labs the morning of 11/22/2023.

3. At 1740 hours on 02/07/2024, per interview with S5, Risk Manager, confirmed no report was completed on 11/22/2023 detailing a patient altercation with a staff member. S5 confirmed this as a reportable event, and that no information was available about the incident.
