The facility failed to ensure maintain documentation regarding transfer times from normal power to generator power. The maintenance staff was unaware of the requirement to maintain this documentation. This deficient practice resulted in the increased risk of the generator not being able to transfer power from normal power to generator power in less than 10 seconds. The facility census was 18 at the time of survey.

Findings are:

1. Record review on 1-29-2019 by State Fire Marshal staff revealed that the facility failed to document the time to transfer on the generator inspector reports.

2. Interview on 1-29-2019 by State Fire Marshal staff with Maintenance Staff A confirmed that this documentation had not been completed and that the maintenance staff was not aware of the requirement to document this information.
