Based on interview and document review, the facility failed to screen all hospital staff for infection and infectious disease risk.
Findings:
2 medical staff credential files were reviewed on 10/4/17. 1 of the 2 credential files reviewed did not contain a current health assessment or evidence of a current PPD. The physician was reappointed in 2016.
Interview with the Director of Quality on 10/4/17 could not provide the required documentation.
