Exit interview with maintenance representative #1 and maintenance representative #2 on March 10, 2014, between 1:35 PM and 1:45 PM, confirmed the facility exceeds the maximum allowable story height for this type of construction and identified the facility has an acceptable FSES conducted on March 10, 2014, addressing this issue.

Based on observation and interview, it was determined the facility failed to maintain one corridor door, affecting one of nine smoke compartments in this component.
Findings include:
1. Observation on March 10, 2014, at 11:40 AM revealed resident room 218 corridor door did not provide positive latching when closed in its frame.
Exit interview with maintenance representative #1 and maintenance representative #2 on March 10, 2014, between 1:35 PM and 1:45 PM, confirmed the door did not latch.

Based on observation and interview, it was determined the facility failed to maintain one exit stair tower enclosure, affecting two of nine smoke compartments in this component.
Findings include:
1. Observation on March 10, 2014, between 11:47 AM and 1:12 PM, revealed the following unprotected vertical penetrations:
a. 11:47 AM - Third floor landing, of the main entrance exit stair tower, had an unprotected penetration around a sprinkler pipe above the suspended ceiling.
b. 1:12 PM - Second floor, two unprotected penetrations through the floor slab above the old nursery.
Exit interview with maintenance representative #1 and maintenance representative #2 on March 10, 2014, between 1:35 PM and 1:45 PM, confirmed the unsealed penetrations.

Based on observation and interview, it was determined the facility failed to maintain one smoke barrier, affecting two of nine smoke compartments in this component.
Findings include:
1. Observation on March 10, 2014, at 12:05 PM revealed the smoke barrier wall, located near room 208, had an unsealed penetration above the suspended ceiling.
Exit interview with maintenance representative #1 and maintenance representative #2 on March 10, 2014, between 1:35 PM and 1:45 PM, confirmed the unsealed penetration.

Based on observation and interview, it was determined the facility failed to properly maintain one sprinkler, affecting one of nine smoke compartments in this component.
Findings include:
1. Observation on March 10, 2014, at 1:22 PM revealed a sprinkler head, located in the first floor central supply room, was obstructed by two metal shelving racks located less than 18 inches below the sprinkler head.
Exit interview with maintenance representative #1 and maintenance representative #2 on March 10, 2014, between 1:35 PM and 1:45 PM, confirmed the sprinkler head obstruction.
