Based on review of personnel files, review of job description and interview, the facility failed to ensure current certifications for two employees.
The findings included:
Review of personnel file of employee #2, revealed the employee's Pediatric Advanced Life Support (PALS) expired March, 2013.
Review of personnel file of employee #3, revealed the employee's Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Cardiac Life Support (PALS) expired March, 2013.
Review of the Emergency Department Registered Nurse (RN) job description, with a revision date of May 9, 2012, revealed "...current certification in ACLS, BLS (basic life support), and PALS..."
Review of the Emergency Department Paramedic (EMT-P) job description, with a revision date of May 22, 2012, revealed "...current certification in BLS, ACLS and PALS..."
Interview with the Emergency Department manager, on May 22, 2013, at 1:30 p.m., in the conference room, confirmed the RN's PALS certification was expired and the Paramedic's ACLS and PALS certification was expired.
