Beaumont Campus
A life safety code (LSC) survey was conducted on site on 2/12/2019. An entrance conference was held for the Plant Operations Manager on 2/12/2019 to discuss the purpose of the LSC survey, and the survey process was explained. Instructions were provided on writing plans of correction. An opportunity was provided for questions and discussion.
The LSC survey was conducted under the authority of Chapter 2 of the State Operations Manual, Appendix A, Acute Care Hospitals, 42CFR482 to determine the Hospital's compliance.
An LSC exit conference was conducted on 2/12/2019 in the hospital. In attendance were the Plant Operations Manager and the Vice President. The preliminary findings of the survey and the next steps in the survey process were explained. An opportunity was provided for questions and discussion.
Note: The CMS-2567 (Statement of Deficiencies) is an official, legal document. All information must remain unchanged except for entering the plan of correction, correction dates, and the signature space. Any discrepancy in the original deficiency citation(s) will be reported to the Dallas Regional Office (RO) for referral to the Office of the Inspector General (OIG) for possible fraud. If information is inadvertently changed by the provider/supplier, the State Survey Agency (SA) should be notified immediately.

The inspector observed, while accompanied by the Plant Operations Manager during the hours of the inspection from 1:00 pm to 4:30 pm on 2/12/2019 that there were the following issues. Documents for Flame Spread Ratings for interior wall and ceiling finishes, draperies, curtains, loosely hanging fabrics, and upholstered furniture and mattresses were not available for review.

The inspector observed, while accompanied by the Vice President during the hours of the inspection from 9:00 am to 3:30 pm on 2/13/2019 that there were the following issues. They were: In the kitchen, there was a Type ABC Fire extinguisher too close to the kitchen cooking grill. Only Type K portable fire extinguishers are allowed to be mounted within 25 feet of any cooking location that uses combustible cooking fluids.

They were: On the fifth floor, in the equipment/electrical room there was a table stored in front of the electrical panel box. This area cannot be used for storage.
"Dedicated Equipment Space: All switchboards, panelboards, distribution boards, and motor control centers shall be located in dedicated spaces and protected from damage." - NFPA 70, 2012, 110.26(E)

There were items stored in the electrical room 1093C in front of the electrical panels. Items may not be stored in front of these panels per the following.
