Based on medical record review, document review and interview, nursing staff did follow facility policy. Specifically, the use of a Pure wick external catheter was not documented in the medical record and the device was left in place/not removed upon Patient #1's discharge from the Emergency Department (ED).
Findings Include:
Review of the policy "External Female Catheters" dated 05/2021 revealed no evidence of where to document use of the catheter in the medical record. Assess periodically for external catheter placement. Replace device every 12 hours if soiled and dispose of the device after each use.
Review of the ED medical record for Patient #1 dated 05/26/22 revealed no documentation related to the application and/or monitoring of the Pure wick urinary system.
Interview with Staff (B), ED RN Senior Nursing Manager on 01/12/22 at 01:30 PM revealed no order is required for use of Pure wick system (external catheter). It is a nursing intervention and is used with elderly patients with rule out fracture workups. It is documented on the " avatar (a pictogram of the anatomical man/woman). "
Review of the patient complaint related to Patient #1 revealed a review was completed by the facility on 06/21/22. The facility found that "the lack of removal of the Pure wick was an oversight and additional education will be provided for the staff regarding use of such device."
