Based on record review and interview, the hospital failed to conduct checks on the adult medical-surgical crash cart each shift per procedure. This failed practice had the potential to affect all inpatients that might require emergency medical interventions with equipment not known to be functioning properly.
Findings:
The surveyors toured the Medical-Surgical unit on 06/08/22 and reviewed contents of a binder on top of the "adult crash cart". The surveyors reviewed multiple documents titled "Adult Crash Cart Checklist" and noted the following:
1. The form dated January of 2022 lacked documentation of lock and defibrillator checks for 0700-1900 shift on the 7th, 16th, and 23rd through the 27th of January; and lacked documentation of lock and defibrillator checks for 1900-0700 shift on the 3rd and 8th of January.
2. The form dated February of 2022 lacked documentation of lock and defibrillator checks for 0700-1900 shift on the 4th and 10th of February; and lacked documentation of defibrillator checks for 1900-0700 shift on the 2nd and 21st of February.
3. The form dated March of 2022 lacked documentation of lock and defibrillator checks for 0700-1900 shift on the 14th of March; and lacked documentation of lock and defibrillator checks for the 1900-0700 shift on the 2nd, 12th, and 16th of March.
4. The form dated April of 2022 lacked documentation of lock and defibrillator checks for 0700-1900 shift on the 3rd, 8th, 16th, and 18th of April; and lacked documentation of lock and defibrillator checks for the 1900-0700 shift on the 8th and 13th of April.
5. The form dated May of 2022 lacked documentation of lock and defibrillator checks for the 0700-1900 shift on the 3rd and 10th of May.
6. The form dated June of 2022 lacked documentation of lock and defibrillator checks for the 0700-1900 shift on the 7th of June.
During an interview on 06/09/22, Staff C stated the facility did not have a policy that specifically addressed checks of the crash carts, but the task was the responsibility of the charge nurse each shift to complete.
