Based on observation and staff interview, the facility failed to maintain hazardous areas in a manner that would retard the spread of smoke to adjacent areas in the event of fire in hazardous areas. Findings:
1. A tour of the facility was conducted on 06/14/2022 and the following observations were made:
a. Unsealed wall penetrations were observed in mechanical closet #7.
b. Unsealed wall penetrations were observed in mechanical closet #8.
2. The maintenance supervisor was present during the tour of the facility and acknowledged the problems with the hazardous areas. He stated he would make the repairs as soon as possible.

Based on observation and staff interview, the facility failed to install manual fire alarm pull stations accessible to all residents by locating them between 42 inches and 54 inches above floor level in accordance with NFPA 72. Findings:
1. A tour of the facility was conducted on 06/14/2022 and the following observations were made:
a. The manual fire alarm pull stations located on the third and fourth floors were mounted at 68 inches above floor level.
b. The maintenance supervisor was present during the tour and stated he would have the pull stations lowered.
NFPA Standard: NFPA 72, 2.8.1, requires all pull stations be located between 42 and 54 inches above floor level.

Based on observation and interview, the facility failed to maintain smoke barrier walls to provide at least a one half hour fire resistance rating. Findings:
1. A tour of the facility was conducted on 06/14/2022, and the following observations were made:
200 and 400 smoke wall
a) Several unsealed penetrations, were observed above the fire doors in the attic, on both sides of the wall.
2. The maintenance director was present during the tour of the facility and he acknowledged the unsealed penetrations in the smoke walls.
