Based on record review and interview, the facility failed to ensure a Baker Acted patients is discharged to a facility that could provided appropriate post-hospital care, for one of ten sampled patients (#1).


Findings:

Review of the facility's policy, "Discharge Planning Generalized," revised 08/14, state that the development of the plan of care and implementation is made through matching available resources with the defined patient's needs.

Sampled patient (SP) #1 was Baker Acted by law enforcement on 04/23/15. On 04/23/15 at 4:14 PM, the patient was deemed incompetent to consent for treatment. The admission notes on 04/23/15 the patient is intellectually challenged, and cognitively impaired. The notes further showed a guardian was appointed. On 05/12/15, the Social Worker (SW) notes showed that the Independent Living provider was working with [named] administrator of Facility #3 a group home to arrange payment for SP#1 placement. The SW was told that facility #3 was not a viable option. The SW notes on 05/19/15 at 11:39 AM then noted the patient is to go to a shelter. Discharge instructions on 05/19/15 at 11:41 AM showed that the patient was discharged to a shelter via van.

On 06/09/15 at 2:09 PM, the Director of Case Management stated that they do not transfer patients to shelter because of lack of funding. She stated that they look at the appropriateness of the patient. She stated that a group home would have been appropriate.
