Means of Egress Capacity

Tag No.: K0231

Based on observation and interview, it was determined the facility failed to maintain capacity of exits.

Findings include:

Observation on July 12, 2021 at 11:13 a.m. revealed the following that the patient care area on the second floor patient care area there were 7 soiled cart, 3 work stations plugged into the wall, and 3 Blood pressure machines plugged into the wall, were being stored in the corridor.

Interview with maintenance director 1 on July 12, 2021 at 11:32 a.m. confirmed the above conditions existed.

Illumination of Means of Egress

Based on observation and interview, it was determined the facility failed to maintain illumination of means of egress.

Findings include:

Observation on July 13, 2021 at 9:48 a.m. revealed the MRI corridor lights can be manually shut off leaving the area with no working lights in the corridor and after interview maintenance director stated no lights in the area are tied into emergency power.

Interview with maintenance director on July 13,2021 at 9:49 a.m. confirmed the information above.

Sprinkler System - Maintenance and Testing

Based on observation and interview, it was determined the facility failed to ensure that automatic sprinkler systems were inspected and maintained, piping free of external loads, sprinkler heads free of build up of dirt and debris, maintain sprinkler and escutcheons, on two of three levels.

Findings include:

A. Observation on July 12, 2021 revealed:

1. Observation on July 12, 2021 at 10:57 a.m., on 1st floor mechanical equipment room, above air handler 3, had wires zipped tied to sprinkler pipe;

2. Observation on July 12, 2021 at 11:45 a.m., on 1st floor sleep lab employee lounge has paint on the sprinkler bulb and head;

3. Observation on July 12, 2021 at 11:47 a.m., on room 3/4 had paint on the head of the sprinkler bulb and head;

4. Observation on July 12, 2021 at 1:00 p.m., 1st floor, OR work room, had a sprinkler escutcheon pulled away from ceiling.

Interview with the maintenance director on July 12, 2021 at 1:00 p.m. confirmed the above deficiencies as noted.

B. Observation on July 13, 2021 revealed:

1. Observation on July 13, 2021 at 9:44 a.m., hospital side tunnel, had paint on a sprinkler head;

2. Observation on July 13, 2021 at 10:36 a.m., Education Center, had wires on sprinkler pipe.

Interview with the maintenance director on July 13, 2021 at 10:36 a.m. confirmed the above deficiencies as noted.

Electrical Systems - Other

Tag No.: K0911

Based on observation and interview, the facility failed to maintain and inspect electrical system requirements per NFPA 70 and NFPA 99, on one of one floors.

Findings include:

Observation on July 13, 2021, at 9:37 a.m., revealed the MRI equipment room, had a junction box that was missing the cover plate.

Interview with the maintenance supervisor on July 13, 2021, at 9:37 a.m., confirmed the the junction box was missing the cover.

Electrical Equipment - Power Cords and Extens

Tag No.: K0920

Based on observation and interview it was determined the facility failed to ensure that electrical wiring and equipment was maintained and the improper and unauthorized use of power strips, extension cords and outlet multipliers are prohibited on one of three levels within this component

Findings include:

1. Observation on July 12, 2021 at 10:55 a.m 2nd floor mechanical room above air handler 1299 revealed the cover missing off of a 4x4 electrical box cover

2. Observation on July 12 at 11:10 a.m. revealed there was a UL power strip permanently mounted on the wall

Interview with Maintenance director on July 12 at 11:12 a.m. confirmed the above deficiencies.

1. Observation on July 13, 2021 at 9:19 a.m., revealed the ER nurses station, had a power strip permanently mounted under a desk.
