Based on interview and review of the facility organizational chart the facility failed to ensure the supervision and evaluation of the clinical activities for the contracted dialysis services was being conducted by a qualified hospital employed registered nurse (RN).
Failure to have a nurse at the facility with direct oversight for the dialysis care provided by the contracted agency potentially puts patients at risk for not receiving adequate dialysis services.
Findings include:
1. Review of the facility organization chart dated May 2016 revealed the person responsible for the day to day operations was not a licensed nurse. The person responsible for the day to day operations was a respiratory therapist who reported to a pharmacist who would then report issues to a licensed nurse.
2. On 5/31/2016 at 10:00 AM the director of dialysis services (staff A) was interviewed. The director was a respiratory therapist. The director stated " I oversee the day to day operations. I report to my manager who is a pharmacist and my manager reports to their director which is a nurse if problems are discovered with the dialysis services ".
