Based on record review and interview, the facility failed to ensure a registered nurse (RN) evaluated the nursing care for each patient in accordance with the patient's needs, in that, 5 of 7 inpatient records (Patient #8, #9, #10, #11, and #18) did not document a 24 hour assessment by a registered nurse during each inpatient day.

Findings included

Patient #8's record did not document a 24 hour assessment by a registered nurse during 10/11/16 and 10/12/16 inpatient days.

Patient #9's record did not document a 24 hour assessment by a registered nurse during 10/11/16 and 10/12/16 inpatient days.

Patient #10's record did not document a 24 hour assessment by a registered nurse during 10/10/16, 10/11/16, and 10/12/16 inpatient days.

Patient #11's record did not document a 24 hour assessment by a registered nurse during 10/07/16, 10/08/16, 10/09/16, and 10/10/16 inpatient days.

Patient #18's record did not document a 24 hour assessment by a registered nurse during 10/11/16 and 10/12/16 inpatient days.

During an interview on 10/13/16 ending at 10:30 AM, Personnel #12 was informed of the above findings and asked to confirm. Personnel #12 confirmed there were no RN assessments documented for the above listed patient days.
