Based on observation, it was determined that, the facility failed to have exits that are readily accessible at all times.
Findings include:
1. On 4/6/10, at 10:30 AM, in the presence of Staff #3, the exit door to the courtyard adjacent to the P7 elevator did not immediately open.

Based on observation, it was determined that, the facility failed to provide exit corridors of 4 feet width(clear and unobstructed).
Findings include:
1. On 4/6/10, at 11:30 AM, in the presence of Staff #3, in the 2nd floor corridor between 2B and Old Main units, the exit way width was restricted to 2'-6" by stretchers, wheelchairs, and boxes.

Based on observation, it was determined that, the facility failed to provide emergency lighting.
Findings include:
1. On 4/6/10, at 10:00 AM, in the presence of Staff #3, the battery powered light in the in the 2nd generator room could not light.

Based on observation, it was determined that, the facility failed to display exit lights with continuous illumination.
Findings include:
1. On 4/6/10, at 10:15 AM, in the presence of Staff #3, in the chapel, the 2 exit lights did not light.

Based on observation, it was determined that, the facility failed to provide a Type 1 Essential Electrical System in accordance with NFPA 99.
Findings include:
1. On 4/6/10, at 10:40 AM, in the presence of Staff #3, the generator in the 1st generator room was a natural gas fired unit which did not have a fuel source on site.

Based on observation, it was determined that, the facility failed to store flammable liquids in approved containers.
Findings include:
1. On 4/6/10, at 10:35 AM, in the presence of Staff #3, in the 1st generator room, there were 23 gallons of flammable xylene that were not stored in a fire rated cabinet.
