Based on observation and staff interview, it was determined that the facility failed to ensure that all exits be maintained visually apparent to all occupants.

Findings include;

1. On 12/2/15 at approximately 1:00 PM in the presence of Staff #48, an illuminated exit sign was not found within the Radiology Department Control Room to inform the occupant of the most expeditious means of exiting in the event that it was needed.

Based on observation and staff interview, it was determined the facility failed to ensure that all means of egress remain free of obstruction or impediment.

Findings include:

1. On 12/2/15 at approximately 1:45 PM in the presence of Staff #48, in the Emergency Department Fast Track near the Orthopaedic Storage Room, the hallway was observed to be congested with medical equipment that would impede exiting in the event of an emergency.

2. This finding was confirmed by Staff #48.
