Based on interview, the maintenance personnel lacked documentation of education for maintenance of facility equipment for 1 of 1 maintenance personnel files (P57).
Findings Include:
1. Interview with P57, Maintenance on 11/20/17 at 12 noon confirmed lack of training when first started job in July, but had contractors come and train him and lacked documentation of training.
2. Interview with P52, Interim Chief Executive Officer on 11/20/17 at 3:15 pm confirmed P57 personnel file lacked documentation of completion of education on boilers and hot water heaters and other equipment.
