Based on observation and an interview with staff # 9 during a tour of the inpatient snack area on the afternoon of 6/20/2016 multiple expired food snacks were observed and available for patient use.
Findings were
During a tour of the inpatient snack area the following expired food items were observed
4 cans ensure dietary supplement expired 3/2016
68 packets of thicken up expired 8/25/2015
2 Breeze Nutritional Drink expired 5/7/2016
1 Breeze Nutritional Drink expired 4/18/2016
1 Breeze Nutritional Drink expired 4/14/2016
In an interview during the tour with staff #9 on the afternoon of 6/20/16, staff # 9 agreed with the findings.

Based on observation and an interview with the Director of the Physical Therapy department staff # 20 on the afternoon of 6/20/2016; the facility failed to ensure the safety of patients. There was no evidence that the temperature of the hot packs in the hydrocollator were checked prior to patient use which could cause injury to a patient.
Findings were;
During the tour of the physical therapy department on the afternoon of 6/20/16 there was no evidence of the hydrocollator hot packs temperature being checked prior to patient use.
Review of policy Hot Packs-Hydrocollator Pack stated, " Disadvantages of hot packs little or no temperature control. "
In an interview with staff # 20 the Director of Physical Therapy department during the tour on the afternoon of 6/20/16, staff # 20 stated they did not check the temperature of the hot packs prior to patient use.
