Fire Alarm System - Testing and Maintenance

Based on observation and interview, it was determined the facility failed to maintain the automatic fire alarm system in one instance, affecting one of nine smoke compartments.

Findings include:

1. Observation on June 9, 2021 at 12:55 p.m., revealed the facility failed to maintain a smoke resistive ceiling for the proper operation/activation of the fire alarm system. There were multiple missing ceiling tile in the first floor IT server room.

Interview with Facility Administration and Maintenance Staff on June 9, 2021, at 1:00 p.m., confirmed the smoke detection deficiency.

Sprinkler System - Maintenance and Testing

Based on observation and interview, it was determined the facility failed to maintain the automatic sprinkler system in one instance, affecting one of nine smoke compartments.

Findings include:

1. Observation on June 9, 2021 at 10:55 a.m., revealed multiple penetrations in the ceiling of the IT Room in the basement, potentially affecting sprinkler performance.

Interview with Facility Administration and Maintenance Staff on June 9, 2021, at 1:00 p.m., confirmed the smoke detection deficiency.

Subdivision of Building Spaces - Smoke Barrie

Based on observation and interview, it was determined the facility failed to maintain smoke barrier doors in one instance, affecting two of six smoke compartments.

Findings include:

1. Observation on June 9, 2021, at 11:50 a.m., revealed the smoke barrier doors leading into the ORs, equipped with latching hardware, failed to fully close and latch when tested.

Interview with Facility Administration and Maintenance Staff on June 9, 2021, at 1:00 p.m., confirmed the smoke barrier doors failed to close and latch.

Electrical Systems - Receptacles

Based on observation and interview, it was determined the facility failed to maintain electrical receptacles for three of over 200 receptacles inspected.

Findings include:

1. Observation on June 9, 2021, revealed the following electrical outlets, within six feet of a sink, were not GFCI protected:

a) 12:35 p.m., there were two receptacles in the first floor biohazard room across from the nurse station;
b) 12:47 p.m., there was one receptacle in the microbiology lab.

Interview with Facility Administration and Maintenance Staff on June 9, 2021, at 1:00 p.m., confirmed the electrical receptacles were not GFCI protected.
