Based on a review of documents and interview the facility failed to ensure that discharge summaries for patients included the disposition of care and provisions for follow-up care.

Findings included:

Facility based policy entitled, "DISCHARGE SUMMARY- PHYSICIAN" stated in part,
"PROCEDURE
The discharge summary will include the following: ...
10. Where patient was discharged or transferred ..."

Review of the medical record for Patient #1 revealed the following:
The discharge summary on 02/19/19 stated in part,
"Transition Plan
Discharge Instruction(s):
Discharge to: Other healthcare institution Patient Condition at Discharge: Stable"

This discharge summary did not reflect the disposition of care including where the patient was discharged/transferred to per facility policy.

In an interview on 07/15/19 staff member #1 verified that the discharge summary for this pateint did not accurately reflect their disposition of care.
