Based on staff interview and record review, the facility failed to ensure staff was familiar with their fire plan per the requirements of:
2012 NFPA 101 Section 19.7.2.3.3
The deficiency affected 1 of 9 required provisions of the fire plan, and has the potential to affect all patients, staff and visitors.
Findings include:
Interview with the Facility Team Leader on 12/6/2016 at 10:05 AM indicated that during an actual fire emergency the staff called 911 and failed to activate the building fire alarm. The facility fire plan, R-A-C-E was not followed, "A" being to activate the building fire alarm.
The maintenance Team Leader was present when the deficiency was identified and acknowledged by the Vice President of Support Services during the exit conference on 12/6/2022.
