Based on observation, it was determined that the facility did not maintain an environment to ensure acceptable level of safety and quality. Specifically, the facility did not maintain walls and equipment in the operating rooms (OR) that could be sanitized.

Findings include:

On the morning of 10/5/2011, a tour of the facilities OR's was conducted. OR 1 was observed to have exposed drywall about waist high along the entire length of the wall making it unable to be sanitized. Additionally, the operating table was observed to have tears in the padding making it unable to be sanitized. OR 2's operating table was observed to have a small tear on the side of the padding making it unable to be sanitized. OR 3's operating table's arm rest was observed to have tears in it making it unable to be sanitized.

Based on interview and review of a humidity report printout, it was determined that the facility was not providing surgical services in a manner that assured the physical safety of all individuals in the area. Specifically, the facility was not logging and tracking humidity and temperature, and keeping the humidity in the appropriate ranges. "Guidelines for Design and Construction, 2010 Edition, Table 7-1", recommends that a ASC's temperature values should be between 68-75 and humidity values between 30-60%.


Findings include:

On the morning of 10/5/2011, the surveyor requested the log for the temperature and humidity levels. The operating room (OR) nurse stated that they do not have a log and they do not track temperature and humidity. She stated that she believes the materials manager (MM) manages humidity and temperature.

An interview was conducted with the MM on 10/6/2011, he was asked who tracks and logs temperature and humidity for the OR, in which he stated that he can control the system electronically for OR 1 and OR 2, but has no way of introducing humidity into OR 3. The MM also stated that he monitors the temperature and humidity but does not track and log it.

Review of a printout for humidity provided by the MM revealed that on 10/6/2011 at 10:35 A.M. the humidity for OR 1 was 25%, OR 2 was 30% and OR 3 was 19%.
