Based on observation the facility failed to provide proper maintenance to the sprinkler system as per NFPA 13 (Standard for the installation of Sprinkler System). Several sprinkler heads had paint on them; this could cause the heads to operate improperly. This deficiency affects all 12 residents in the facility.

Note: NFPA 13, 1999 Edition:
NFPA 13, Chapter 3, " Sprinklers " 3-2.6.3 Unless applied by the manufacturer, sprinklers shall not be painted, and any sprinklers that have been painted shall be replaced with new listed sprinklers of the same characteristics, including orifice size, thermal response, and water distribution.

Findings:

During a tour of the facility on November 22, 2010 between 2:30 p.m. and 4:30 p.m. it was observation that the sprinkler heads in rooms 8, 9, Conference /Office, and Quite Activity had paint on them.

Based on visual observation the facility failed to provide smoking regulations that are in accordance with NFPA 101: 19.7.4., for 1 of 1 location. This deficiency affects all residents that use the designated smoking area, 5 residents were observed using the smoking areas.

Actual Code:
NFPA 101:18.7.4 Smoking regulations shall be adopted and shall include not less than the following provisions:
(1) Smoking shall be prohibited in any room, ward, or compartment where flammable liquids, combustible gases, or oxygen is used or stored and in any other hazardous location, and such areas shall be posted with signs that read NO SMOKING or shall be posted with the international symbol for no smoking.
(2) In health care occupancies where smoking is prohibited and signs are prominently placed at all major entrances, secondary signs with language that prohibits smoking shall not be required.
(3) Smoking by patients classified as not responsible shall be prohibited, unless under direct supervision.
(4) Ashtrays of noncombustible material and safe design shall be provided in all areas where smoking is permitted.
(5) Metal containers with self-closing cover devices into which ashtrays can be emptied shall be readily available to all areas where smoking is permitted.
