Based on a review of the facility records and interview with the staff, it was determined the facility failed to ensure the Heating, Ventilating, and Air Conditioning System (HVAC) was maintained in reliable operating condition.
The findings include:
A revisit survey was conducted on 05/23/12, which revealed the facility had not corrected K-0067. The administrator stated the facility sent a letter to the Office of Plans and Construction on 02-24-12, and have not heard back from them.
