Based on document review, observation and interview, it was determined that for 10 of 10 intravenous medication vials (Verapamil), and 3 of 9 eye ointments (Bacitracin ophthalmic), the Hospital failed to ensure outdated drugs were not available for patient use.
Findings include:
1. The Hospital policy titled, "Operations-Medication Area Inspections" (revised 8/10) required, "Pharmacy Department: 1. Pharmacy areas should be routinely inspected by pharmacy staff. 2. Particular attention is paid to the following: a. Expiration dating...."
2. During an observational tour of the Pharmacy on 5/1/14 at approximately 10:30 AM the following outdated medications were available for patient use.
*2 (10 mg/4 ml) vials of Verapamil with an expiration date of 3/1/14.
*8 (5 mg/2 ml) vials of Verapamil with an expiration date of 3/1/14.
*3 (3.5 gm) tubes of Backtracking ophthalmic ointment with an expiration date of 2/2014.
3. The above findings were discussed with the Pharmacy Director and the Corporate Pharmacy Director on 5/1/15 at approximately 10:45 AM, who stated that medications are inspected for expiration monthly or every other month, and that all the expired medications are removed.
