Based upon observation and interview, it was determined the facility failed to maintain hazardous areas in compliance with regulations.

Findings include:

Observation on August 24, 2010 at 11:45 am revealed that room # 268 was being used as storage, however the room did not meet the requirements for storage. The door lacks an automatic closure.


Interview with the Administrator and Maintenance Director on September 02, 2010 at 9:30 am confirmed that the condition exists.

Based upon observation and interview, it was determined that the exit and directional signs are not in accordance with regulations.

Findings include:

Observation on August 24, 2010 at 11:00 am revealed that there was an exit sign not lit in the corridor beside room # 278

Interview with the Maintenance Director on September 02, 2010 at 9:30 amconfirmed that the condition exists.

Observation and interview, it was determined that facility personnel failed to store medical gas in accordance with regulations in one location throughout the facility.

Observation on August 24, 2010 at 1:30 pm revealed that the oxygen storage room on the 2nd floor did not meet the requirements for the storage of compressed medical gases.
