Based on observation, testing, and interview, the provider failed to maintain the two hour fire resistance rating between buildings at:
*The two hour wall between the hospital and the nursing home boiler room.
*The north leaf of the ninety minute fire rated doors located between the clinic and the hospital.
Findings include:

1. Observation, testing, and interview, on 7/26/11 at 8:40 a.m. with the director of plant operations revealed:
*The north leaf of the ninety minute fire rated doors located between the clinic and the hospital would not latch when released from the magnetic hold open device.
*The door was monitored during monthly fire drills and had latched last month.

2. Observation, and interview on 7/26/11 at 10:45 a.m. with the director of plant operations revealed:
*The two hour wall between the hospital and the nursing home boiler room had unsealed penetrations in the wall.
*Two insulated pipes had been cut off on the nursing home side leaving two open pipes running into the hospital side. The ends of the pipes were open and the penetration in the wall around the pipe was not sealed.
*Two conduits had been cut off on the hospital side. The conduit contained copper wire but was not capped, and the penetration in the wall around the conduit was not sealed.
*That section of wall must have been missed on the final construction walk through. They had looked at another section of the same wall and had the the construction company repair and seal the penetrations. He did not know why they did not complete that section of the wall.

1. Observation, testing, and interview on 7/26/11 at 1:00 p.m. with the director of plant operations revealed:
*The ninety minute fire rated door would not latch into the frame when released from the magnetic hold open device.
*The door was monitored monthly during the fire drills. The door latched last month.
