A. Based on a review of the 77 Illinois Administrative Code 250, a review of certified nurse aide (CNA) files, and staff interview, it was determined that in 2 of 6 (Employees #1, and #2) CNA files reviewed, the Hospital failed to ensure proper background checks were conducted.
Findings include:
1. The 77 Illinois Administrative Code 250, "Section 250.435 Health Care Worker Background Check" was reviewed on 2/18/10. It indicated that, "A hospital shall comply with the Health Care Worker Background Check Act [225 ILCS 46] and the Health Care Worker Background Check Code (77 Ill. Adm. Code 995).
2. The credential files of Employees #1 and #2 were reviewed on 2/18/10. There was no documentation that indicated the State requirements for background checks were conducted in accordance with the State of Illinois requirements.
3. During interview with the CEO and Director of Nursing, conducted on 2/18/10 at 1:45 PM, the above findings were confirmed.

A. Based on clinical record review, a review of internal documentation, and staff interview, it was determined that in 1 of 6 (Pt #1) medical records reviewed, the Hospital failed to ensure all care was provided based on the patient's needs.
Findings include:
1. The medical record of Pt #1 was reviewed on 2/18/10. It indicated that Pt #1 was admitted on 9/29/09 with diagnoses of Anemia, rule out Gastrointestinal Bleed, and Weakness. Nursing documentation, dated 9/30/09 at 2:52 AM, indicated that the patient was to "Transfers with 2:1, Ambulates with 2:1 and again on 9/30/09 at 10:04 PM was identified with the same transfer/ambulation requirements of 2 assistants for the patient. There was no documentation that indicated the patient was transferred with the help of 2 assists.
2. A review of internal documentation was conducted on 2/18/10. It indicated that Employee #6 was counseled on 10/23 related to "Hurried during transferring pt. Observed as rough with transfer." The "Plan for Correction" was, "Employee will ask for assistance and be in a nonhurried attitudeed (sic)." The "Expectations:" were "She will take extra time with patients during transfers."
3. During an interview with the CEO and the Director of Nursing, conducted on 2/18/10 at 2:45 PM, the above finding was confirmed.
