MAINTENANCE OF PHYSICAL PLANT

Based on observation and interview the facility failed to maintain an environment that ensured maintenance of the health and safety of patients resulting in the potential for harm for patients treated in the surgical suite. Findings include:

On 11/15/16 at 1400 during a tour of the surgical suite with staff C found in Operating Room (OR) #1 on the west wall, screw holes that were left unsealed resulting in accumulation of dirt and dust. This surface is not capable of being cleaned.

On 11/15/16 at 1415 found in OR rooms 1-4, at least one ceiling tile in each room that was a standard drop type ceiling that did not meet the requirement of being a cleanable surface.

On 11/15/16 at approximately 1430 found multiple attached power strips that were "daisy-chained" to accommodate multiple electrical device connections. This arrangement of extension cords posed a potential hazard for fire in addition to a risk for fall.

The above mentioned findings were confirmed by staff C at the time they were discovered.

On 11/15/16 at 1500 observed housekeeping staff D cleaning surfaces without the ability to reach those surfaces that were beyond their reach. When asked how they clean high surfaces staff D responded that they used an inverted bucket to stand on to reach tall surfaces.
