The hospital failed to construct and maintained the facility to ensure the safety of the patients with regard to ensuring that vertical openings were protected with at least a one hour fire rated construction and that patient room doors were arranged so that the patients could open the door from inside without using a key.

Based on review of facility documentation, facility tour and staff interview and verification, the facility failed to meet the applicable provisions of the Life Safety Code of the National Fire Protection Association. Potentially all persons utilizing the facility could be affected. The facility had a census of 356 patients at the time of the survey.
Findings included:
The following findings were noted in the main hospital;
K20, which addressed the facility's failure to ensure all vertical openings and fire barriers were protected with at least a one hour fire rated construction in regard to doors not being fire rated.
K43, which addressed the facility's failure to ensure that patient room doors were arranged so that the patients could open the door from inside without using a key.
