Based on policy review, medical record review, and staff interview, the facility failed to ensure restrained patients were monitored per their policy for one of two restrained patients whose medical records were reviewed (Patient #1). A total of ten medical records were reviewed. The facility census was 68.

Findings include:

Review of the policy titled, "Restraints," revised 05/08/20, revealed restrained patients would be monitored at least every two hours.

Review of the medical record for Patient #1 revealed orders dated 01/18/22 at 6:30 AM and 01/21/22 at 6:30 AM for bilateral soft wrist restraints. The "Restraint, Non-Violent Documentation" form dated 01/18/22 lacked documentation of restraint monitoring from 8:00 AM until 12:00 PM. The medical record lacked documentation of restraint monitoring on 01/21/22.

This was verified in an interview on 03/16/22 at 10:20 AM with Staff A.
