Based on the review of personnel files, job descriptions, and staff interview, it was determined that the facility failed to employ personnel who possessed the education, experience, and training as outlined in the job description for one Patient Care Technician (PCT) (PCT DD) of six personnel files reviewed (Intensive Care Unit Director (ICUD) AA, Chief Nursing Officer (CNO) BB, Registered Nurse (RN) CC, PCT DD, PCT FF, PCT JJ).


Findings include:


A review of the facility's Job Description for a Patient Care Technician revealed that PCT needed to have up to six months of work-related experience to work in the Intensive Care Unit.


A review of the personnel file failed to reveal that PCT DD had the required work-related experience or education as outlined in the job description.


An interview took place in the facility's conference room on 2/18/25 at 4:00 p.m. with the Director of the Intensive Care Unit Director (ICUD) AA who stated that Patient Care Technicians (PCTs) went through on-the-job training and were not required to have the education or experience as a technician. ICUD AA stated that the new employees had a preceptor for a minimum of three weeks and a maximum of 90 days, which could be extended depending on the strengths and weaknesses of each employee, and how quickly the employees could learn new skills.

Intensive Care Unit Director (ICUD) AA stated that she was not paying attention to PCT DD's Patient Tech Skills Checklist, which ICUD AA documented was signed off on 12/17/25; however, the preceptor and orientee PCT DD had signed off on all the skills on 12/17/24. ICUD AA stated that PCT DD should not have a skills check list signed off yet because PCT DD was still under the 90-day orientation, and ICUD AA stated that she took full responsibility for the documented error.
