MONITORING RADIATION EXPOSURE

Based on surveyor observation, record review and staff interview, it has been determined the facility failed to ensure that radiation workers for 3 of 3 off site hospital X-ray centers were checked periodically, by the use of exposure meters or badge tests, for amount of radiation exposure.

Findings are as follow;

1. Surveyor observation at X-ray center in Woonsocket on 7/30/2019 at approximately 10:00 AM revealed a radiation worker (Staff A) not wearing exposure meter or badge test. Interview during the observation, Staff A confirmed that she has been working at this center and she does use an exposure meter or badge test.

2. Surveyor observation at X-ray center in Cumberland on 7/31/2019 at approximately 10:00 AM revealed another radiation worker (Staff B) not wearing exposure meter or badge test. Interview during the observation, Staff B confirmed that she has been working at this center and she does not use an exposure meter or badge test.

3. Surveyor observation at X-ray center in Smithfield 8/1/2019 at approximately 11:00 AM revealed another radiation worker (Staff C) not wearing exposur meter or badge test. Interview during the observation, Staff C confirmed that she has been working at this center and she does not exposure meter or badge test.

During an interview on 8/2/2019 at approximately 10:00 AM, the Director of Radiology Department revealed all workers at the above off site locations do not wear exposure meters or badge tests. The Director of Radiology Department was unable to provide evidence that these radiation workers have been checked periodically for amount of radiation exposure.
