Based on observation and staff interview the facility failed to ensure the name and phone number of the state contact to file a complaint or grievance was prominantly displayed in an area where the patient's representative or a visitor could see the number and address.

The findings include:

Observation in the waiting area of the hospital admission and front entrance area revealed that the facility did not include the name and phone number of the state contact person to file a complaint or grievance in their other posting information of patient rights. This information was not displayed in a prominent location where a patient's representative or visitor could see the information.

In an interview with the CEO and Chief Nursing Officer of the facility they confirmed the above findings at the time of the findings.
