Based on a observations during a tour of the hospital and interviews with hospital staff it was determined that the the hospital-wide infection control program failed in the following cases:

On the morning of 9/21/2011 , a housekeeper was observed cleaning a bathroom for a patient on Contact Precautions. The housekeeper was not wearing a gown but did have on gloves. Once she was aware that she was being observed she came out used hand sanitizer put on a gown and gloves and again used hand sanitizer and proceeded to clean the rest of the room.

A soiled utility room on six east was also used for the storage of the Blanket warmer and for the storage of the portable electronic thermometer.

The clean laundry sorting room lacked a handsink or liquid hand sanitizer dispensers for staff use.

Linen chute rooms in the basement area lacked storage of any gowns or gloves for staff use as they empty the bags of linens from the rooms. the rooms also lacked a handsink or liquid hand sanitizer dispenser as observed on 9/22/2011.
