Based on observation, the facility failed to assure all applicable Life Safety Code requirements are met regarding fire proofing and fire stopping material in isolated areas of the hospital.

1. Per observation on 1/14/14, there was a lack of fire stopping at pipe penetrations at the rear of room G45E. All pipe, cable or conduit penetrations must be effectively fire stopped. (LSC Sec. 8.3.5)

2. Per observation on 1/14/14, there was evidence that the spray applied fire proofing of steel structural members in some isolated areas of the facility had been removed. Spray applied fire proofing must be restored by a competent contractor. (LSC Sec. 8.3.1.2)
