Based on visual observation the facility failed to assure that all doors within an exit passageway were held open by an approved means. When doors to stairwells, smoke barriers, horizontal exits or hazardous areas are propped open it provides an opportunity to allow fire and/or smoke to flow freely throughout the facility. This deficient practice has the potential to affect 20 of 20 residents.
Findings:
During the facility tour, between the hours of 9:00 am and 3:00 pm on 10/9/18, it was noted that cross corridor doors behind the lobby area did not shut fully when fire alarm activated and the doors on hold open devices did not close properly.
Interview with maintenance operations revealed the facility was not aware that the door was not releasing doors when fire alarm was activated.

Based on visual observation the facility failed to assure that the fire extinguishers were properly charged in accordance with the Life Safety Code and NFPA 10. Fire extinguishers are available to extinguish small fire or smoke emergencies. This deficient practice could potentially affect 20 of 20 residents.
Findings:
During the facility tour and the record review, between the hours of 9:00 am and 3:00 pm on 10/9/2018, it was noted that fire extinguishers were recently inspected but a slow leak was occurring in 2 out of 3 fire extinguishers in facility.
Interview with maintenance supervisor revealed the facility was not aware that the fire extinguishers were leaking after inspection was done on the fire extinguishers.

Based on visual observation the facility failed to provide corridor doors that were not roller latching doors. The deficient practice had the potential to affect 20 of 20 residents. One of one corridor had doors that were deficient.
Findings:
During the facility tour, between the hours of 9:00 am and 3:00 pm 10/9/2018 it was discovered that all doors to sleeping rooms had roller latches between sleeping rooms and corridors.
Interview with maintenance supervisor revealed the facility was not aware of the doors to sleeping could not have roller latches on doors.
