Based on findings from observations and interview, the exit on the north end of the emergency department (ED) was not clearly designated as an exit.

Findings include:

--Per observation on 7/12/11 at 11:32 a.m., the exit at the north end of the ED was not clearly marked with an illuminated exit sign. The Director of Plant Operations was present at the time the observation was made and verified that, because the patient curtains were closed, the existing exit sign could not be seen.

Based on findings from observations and interview, the doors on 4 housekeeping rooms in the Hospital were not equipped with self-closing devices, there were penetrations through the walls and ceiling of the general storage/laboratory closet located adjacent to the hospital laboratory, and, although sufficient medical records were being stored in the administrative area of the outpatient clinic to constitute a hazardous area, the area was not fire protected.

Findings include:

--Per observations on 7/12/11 at 11:00 a.m., the doors to housekeeping rooms 247, 219, and 248, were not equipped with self-closing devices. This was also observed at 1:30 p.m. at the basement housekeeping room. The Director of Plant Operations was present at the time the observations were made and verified that the doors were not equipped with self-closing devices.

--Per observations on 7/12/11 at 11:17 a.m., there were penetrations through the walls and ceiling of the general storage/laboratory room. The Director of Plant Operations was present at the time that the observations were made and verified that there were penetrations present.

-- Per observation on 7/12/11 at 2:30 p.m., approximately 200 linear feet of records on open shelving were being stored in the administrative area of the Outpatient Clinic. Because they are combustible, records that are not contained in a fire rated construction area and/or protected by sprinklers pose a fire hazard.
