The facility did not ensure that exit access is arranged so that exits are readily accessible at all times as required by section # 19.2.2.2.4 of the referenced, "Life Safety Code ".
On 08/02/13 at 2:30 PM, the surveyor while accompanied by the facility Fire Chief observed that adequate provisions are not in place at the Merritt Building for the rapid removal of occupants by a reliable means available to staff at all times, as required by section # 19.2.2.2.5; i.e. during a fire drill it took the nursing supervisor on the 2nd floor unit over 2 (two) minutes to unlock the key vault with exit stair enclosure keys and over 2 ? (two and one-half) minutes for staff to collect the 1st key while staff working near the exit doors carried no keys for the locks on the exit doors. An interview with one staff member revealed that he thought he had a key and spent time looking for it until another staff member told him he did not have one.

The facility did not ensure that fire drills were held at unexpected times under varying conditions at least quarterly on each shift as required by section # 19.7.1.2 of the referenced, " Life Safety Code "
1. On 08/02/13 at 2:30 PM, the surveyor while accompanied by the facility Fire Chief observed that the use of a code phrase to ensure the transmission of an alarm was not used during a fire drill on the 2nd floor, nursing unit at Merritt Building, as required by section # 19.7.2.3 of the referenced, " Life Safety Code " ;i.e. facility has no coded word for fire and uses no coded word for fire during fire drills;
2. On 08/02/13 at 1:00 PM, the surveyor was not provided with documentation from the facility Fire Chief or the Director of Accreditation & Regulatory Compliance to indicate that the fire drills at the Merritt Building are being conducted in accordance with section # 19.7.1 of the " Life Safety Code " ; i.e. these conditions include:
a. All of the 1st shift fire drills at the Merritt Building were all conducted between 9:47 and 10:16 AM;10:25, 9:47, 10:13 & 10:16 AM;
b. All of the 3rd shift fire drills at the Merritt Building were all conducted between 6:15 and 6:36 AM;6:15, 6:35, 6:36 & 6:25 AM.
