Based on review of documentation and interview, it was determined that no individual or group therapy was provided to the patients on the Geriatric Unit during the week of March 2, 2015.

Findings were:

Review of medical records of Geriatric patients 1 through 5, revealed no documented evidence of individual or group therapy during the week of March 2, 2015. This finding was confirmed by the Administrator in an interview on March 16, 2015.
