Based on a review of facility documentation, medical records (MR) and interviews with staff (EMP), it was determined that the facility failed to provide the patient or the patient's representative a copy of the Important Message from Medicare no more than 2 days prior to discharge and at least four hours before discharge for three of three Medicare patient records reviewed (MR1, MR2 and MR3).
Findings include:
A review of the York Hospitals "Medicare Discharge Notice" revised 7/1/15 revealed, "...If the patient's length of stay is greater than 3 days, obtain a new IMM and review it with the patient/family within 48 hours of an anticipated discharge and a signature is required on the medical record copy. ..."
A review on July 21, 2015, of MR1, MR2 and MR3 revealed that the Important Message from Medicare notice had not been provided to the patients at least 48 hours prior to discharge.
An interview conducted on July 21, 2015, at 12:30 PM with EMP4 confirmed there was no evidence in MR1, MR2 and MR3 that the Important Message from Medicare notice was given to the patient no more than two days prior to discharge.
