Based on review of the hospital's policy/procedure, observations, and staff interviews, the hospital failed to ensure patients received care in a safe environment by failing to ensure staff wore facemask at all times.
Findings:

Review of the Hospital Policy titled, "WK Today COVID-19 Blog" dated March 31, 2020 revealed in part: Upon arrival for your shift, you will receive a mask. Wear the mask at all times.

Observation on 09/29/2020 at 1:20 p.m. revealed S2RN without a facemask while seated at a computer station at the 2-north nursing station.
Observation on 09/29/2020 at 1:22 p.m. revealed S3RN without a facemask while seated at a computer station at the 2-west nursing station.
Observation on 09/29/2020 at 1:51 p.m. revealed S4LPN and S5RN without facemask, standing at the 4-Stepdown nursing station.
Observation on 09/30/2020 at 10:30 a.m. revealed S6RN and S7RN without facemask, conversing with one another while standing at the 3-North nursing station.
Observation on 09/30/2020 at 10:35 a.m. revealed S8Physician without a facemask, seated at the 3-North nursing station.

Interview on 09/28/2020 at 8:30 a.m., S1Safety Officer stated staff are to wear their facemask at all times.
Interview on 09/29/2020 at 1:22 p.m., S3RN stated facemask are supposed to be on at all times unless you are eating in a designated area.
Interview on 09/29/2020 at 1:51 p.m., S4LPN and S5RN stated facemask are supposed to be on at all times.
Interview on 09/30/2020 at 10:30 a.m., S6RN and S7RN stated facemask are supposed to be on at all times.
Interview on 09/30/2020 at 10:35 a.m., S8Physician stated he usually has his facemask on.
