Based on record review and interview the facility failed to provide medical information to the receiving facility for 1 (P[patient]1) out of 3 (P1-P3) patients reviewed for discharge planning. The deficient practice could possibly lead to patients not receiving the appropriate care after discharge from the hospital and contribute to patients being readmitted.

The findings are:

A. Record review of P1's medical chart for visit dated 10/19/2024 at 12:56 PM revealed P1 was brought to the hospital via ambulance from a nursing home after a fall.

1. Under "Flowsheets - Care Handoff" a note dated 10/20/2024 at 3:19 AM "Report given to [Name of ambulance company] ambulance." Records indicated that report was provided to the transporting ambulance, but a report was not given to the nursing facility that was going to be providing care to the patient.

2. There was no evidence in the medical chart that the facility provided medical information or verified with the nursing home that the patient was being discharged from the hospital and was able to return to her previous arrangement. There was no evidence that the facility provided medical information to the nursing home regarding P1's visit to the hospital.

B. Record review of P1's medical chart for visit dated 10/20/2024 at 4:19 AM, the provider note stated, "[Patient] came to the emergency department yesterday for fracture dislocation right shoulder and acromion [part of the shoulder blade], who was discharged and taken back to her nursing home where it turned out the nursing home had given away her bed, so she did not have a place to return. The ambulance brought her back. We will now need to have case management find a place for her to live. No new symptoms."

C. During an interview on 04/01/2025 at 9:30 AM with S(staff)5, clinical it was explained that if a patient comes from a nursing home, nursing staff would provide report to the nursing facility when patient is getting discharged.
