Based on interview and review of 1 of 3 clinical record, as part of a complaint investigation, there was a failure to supervise and coordinate pharmacy activities resulting in non availability of medications ordered by physicians.
Findings:
Citing patient #4, a 54 year old male, admitted to the facility involuntary on an emergency detention warrant on 4/7/2010 due to physically assaulting others at the personal care home where he resided. The patient had a diagnosis of paranoid schizophrenia and had a long psychiatric history. Upon admission the patient was psychotic and experiencing visual hallucinations seeing people trying to choke him.
On 4/11/2010 Seroquel XR 600mg for psychosis was ordered to be given daily. However, review of the medication administration record (MAR) reflected Seroquel XR 600mg was not given on 4/11/2010, 4/12/2010, 4/13/2010 and 4/14/2010 . On the MAR it was noted "MNA" for medication not available. The medication not available was verified by personnel ID# 50 at 4: 20pm on 5/18/2010. Personnel #50 also revealed medications not available has been an ongoing problem.
