Based on visual observation the facility failed to provide corridor doors that were not closing, latching and smoke resistive in the frame. When the doors latch a smoke resistive seal is formed to protect the rooms' occupants. The deficient practice had the potential to affect 1 of 21 residents.
Findings:
During the facility tour on 11/8/2017 between the hours of 8:30 am and 12:00 PM observation revealed, corridor door to room 122 door handle missing this door will not latch and know have open hole penetrations.

A remote annunciator is provided in a location readily observed by operating personnel. The annuciator is hard-wired to indicate alarm conditions of the emergency power source.
This STANDARD is not met as evidence by : no annunciator in a constantly monitored location.
The deficient practice have the potential to affect 21 of 21 residents.
Finding: During the facility tour on 11/8/2017 between the hours of 8:30 am and 12:00 PM observation revealed no annunciator panel being monitored at this facility.
