Based on observation, interview and record review, the facility failed to provide sanitary conditions for the storage of food, kitchen equipment and the maintenance of drainage in the three-sink compartment. This has the potential to affect all patients that eat food at the facility. Findings include:

During an observation on 8/12/15 at 10:50 a.m., the three sink compartment did not have an air gap.

During an interview on 8/12/15 at 11:00 a.m., staff member A, facility director, stated he contacted the plumber for the facility and there were no one-way valves or air gaps in the drainage system for the three-sink compartment.

During an observation on 8/12/15 at 10:30 a.m., the following was observed:
-one pan of lasagna was stored in the refrigerator without a seal and exposed to the air;
-one loaf of garlic bread, stored in the refrigerator, was loosely wrapped in foil and exposed to the air;
-four plastic containers of lunch meat were not in the original package, one wrapped block of sliced cheese was not in the original package and one container of cottage cheese not in the original package, that were stored in the back-up salad bar, were not labeled or dated;
-several ice cream toppings not in the original package, stored in the dry goods room, were not dated;
-a scoop was stored inside of the bulk flour and sugar bin;
-the meat slicer and large mixer were stored in high-traffic areas and not covered during non-use.

Review of the facility policy and procedures for food storage and handling reflected scoops were not to be stored in food or ice containers, but kept covered in a protected area near the containers. It also addressed that all foods should be covered, labeled and dated.

Review of the facility policy and procedure for storage of the meat slicer reflected that staff were to reassemble and cover the meat slicer after it was cleaned and sanitized.

During an interview on 8/12/15 at 11:20 a.m., staff member B, dietary manager, stated it was good to have a review and learn what is being missed.
