Based on observation and interview, the facility failed to have hazardous areas separated from the remainder of the building. This practice has the potential to affect 1 of 4 smoke compartments of the facility by allowing smoke, gasses and fire to spread. The facility has a capacity of 18 and a census of 1.
Findings are:
Observation on 7/22/2015 at 12:15 p.m. revealed several dime size holes in the ceiling of the boiler room, close to the West wall.
Observation on 7/22/2015 at 12:18 p.m. revealed three penny size holes in the ceiling of the Janitor Closet by the Employee Entrance.
Observation on 7/22/2015 at 12:23 p.m. revealed penetrations around pipes and cables in the ceiling of the large Storage Room off of Purchasing.
During an interview on 7/22/2015 between 12:15pm and 12:23pm, Maintenance A confirmed all of the findings.
