RN SUPERVISION OF NURSING CARE

Based on record review, document review and staff interviews it was determined the Registered Nurse failed to supervise and evaluate the plan of care for implementation in compliance with the physician ordered plan of treatment for one (#3) of 10 sampled patients.

Findings include:

Patient #3's History and Physical dated March 5, 2015 at 1:59 p.m. was signed by the attending physician. It included documentation indicating Patient #3 was admitted with conditions including generalized weakness and deconditioning.

The Physician Orders dated 3/5/15 at 11:14 a.m. included a physician order for the patient to be evaluated and treated by Physical Therapy.

The Physical Therapy (PT) Initial Evaluation dated 3/5/15 at 12:30 p.m. and signed by the Physical Therapist indicated the plan was to provide daily one on one physical therapy. The PT Note dated 3/6/15 at 2:19 p.m. indicated Patient #3 declined to work with the therapist that day. The next PT Note was dated 3/16/15. A review of all documentation failed to reveal evidence Patient #3 was seen by a therapist or any attempts were made to provide physical therapy services between 3/6/15 and 3/16/15, a period of nine days.

The document titled "Kindred Hospital Central Tampa 2015 Organizational Plan for the Delivery of Patient Care, Treatment, and Services" was reviewed on 4/27/15. Page 5, Section VI. A. included the statement: Interdisciplinary relations are maintained to ensure continuity of patient care services through open communication. Page 8, Patient Care Services, included the statement: The Registered Nurse oversees the coordination of the overall care of the patient...The Registered Nurse coordinates the patient's safe, individualized care and addresses patient and family needs in a way that maximizes available resources.

An interview was conducted on 4/28/15 at approximately 10:00 a.m. with the Chief Clinical Officer, the Director of Rehabilitation Services, and the Physical Therapy Assistant assigned to the care of Patient #3. The Director confirmed the finding Patient #3 was not seen by a therapist or offered Physical Therapy services in compliance with the physician ordered plan of treatment for a period of nine days. The Chief Clinical Officer confirmed the finding the nursing staff failed to take appropriate action to ensure the patient received care and services in compliance with the physician ordered plan of treatment, facility policies, and the interdisciplinary plan of care.
