Based on interview, the facility failed to provide prompt and effective response of healthcare personnel during a fire.

The findings include:

Interview with the Director Facility Management on 9/19/2023 at 10:00 AM revealed, during an actual fire in an open top dumpster located by the loading dock the facility failed to use the code phrase or activate the buildings fire alarm.

The Facility Management Director was present when the deficiency was identified and acknowledged by the Director of Operations during the exit conference on 9/19/2023.
