Based on observations and interviews, the hospital failed to ensure patients received care in a safe setting as evidenced by having nurse call lights on the hand rails of patient beds that were not functioning for 99 of 145 beds located in the hospital. This failure had the potential to delay or prevent assistance from the nursing staff if the patient pressed the call light on the bed hand rail rather than the hand held call light.
Findings:

In an observation on 06/28/2022 at 10:13 a.m. - 10:50 a.m. of the 4th floor cardiac unit revealed patient beds with the nurse call light on the hand rails not functioning.

In an interview on 06/28/2022 at 10:20 a.m. S2Qual stated all patient beds in the hospital have nurse call lights on the hand rails that are not functional because the beds are not connected to the call light system.

In an interview on 06/29/2022 at 5:00 p.m. S1CNO verified there were 99 of 145 patient beds located in the hospital that had nurse call lights on the hand rails that were not functioning.
