Based on observation and staff interview it was determined the facility fire sprinkler system was not installed in accordance to NFPA 13 to provide complete coverage for all portions of the building and was not maintained in accordance with NFPA 25. This could place all occupants at risk in the event of a fire in the facility

The findings were:

During a tour of the facility with staff M on 04/12/12 from 9:00am to 10:30 am it was observed that a wood Bio-Hazard Storage area and a wood over hang were attached to the rear of the main building, but were not provided with fire sprinkler coverage. It was also noted that the two attached canopies one at the Main entrance and the other at the Emergency Entrance which allow vehicular traffic underneath them are not provided with fire sprinkler protection.

During a tour of the facility with staff M on 04/12/12 from 9:00am to 10:30 am it was observed that escutcheons were missing on several fire sprinkler heads throughout the facility.

During a tour of the facility with staff M on 04/12/12 from 9:00am to 10:30 am it was observed that there are two fire sprinkler heads within six feet of each other in the Physical Therapy Office

These findings were confirmed by staff M at the time of discovery.

Based on observation and staff interview it was determined the facility failed to maintain the electrical wiring in accordance with NFPA 70, The National Electrical Code. This could place any staff member working around the areas at risk of electrical shock or possibly electrocution.

The finding were:

During a tour of the facility with staff M on 04/12/12 from 9:00am to 10:30 am it was observed that a large knockout was missing in the side of the MRI Shutoff.

During a tour of the facility with staff M on 04/12/12 from 9:00am to 10:30 am it was observed that a knockout was missing in a junction box in the Assistance Administrator ' s Office.

These findings were confirmed by staff M at the time of discovery.
